1、邀請(qǐng)外商來(lái)連報(bào)告申請(qǐng)表一份，蓋公章

2、邀請(qǐng)單位營(yíng)業(yè)執(zhí)照副本復(fù)印件（如果是投資企業(yè)，還需批準(zhǔn)證書(shū)復(fù)印件）或者代表機(jī)構(gòu)登記證復(fù)印件一份，蓋公章
